Instructions for Athletics Physical & Free Physical Information for 2024-2025 School Year
Physicals are needed to tryout for any sport, along with athletic insurance and completion of the four required NFHS videos.
There is a New Packet for the 24-25 School Year. If you do not pick up your packet from school, please make sure to download the form on the bottom of this page. If you use a physical packet from previous years, they will be invalid, and you will need to complete a new physical.
To ensure you have the correct version of the physical, the bottom right of pages should read the following… FM-3439 Re. (05-24).
The 2024-2025 FHSAA/MDCPS Physical form is designed to be printed on both sides (do not print one side).
We will do our best to have a stack of physical packets at the main office of the high school to pick up. If there are no copies available, please inform the front office so we may make additional copies.
Please review the packet and pay attention to what needs to be signed and notarized before submission.
Physical Examination MUST BE DATED AFTER JUNE 29, 2024 to be eligible for the 2024-2025 school year
Packet MUST be signed, dated, and notarized before you are allowed to receive the free physical examination.
This is different from years past, and there WILL NOT be a notary on campus.
Athletic packets will NOT be available the day of, because you MUST have the form signed, dated, and notarized BEFORE your arrival.
Any missing signatures, dates or the notary will result in your student athlete being delayed in submitting their physical.
Parents: If your child suffers from asthma, epilepsy, seizures or any other chronic medical condition please note that in their physical and advise the athletic trainer on the personalized Emergency Action Plan for your child.
If you are completing the physical outside of school with your PCP or at a clinic, the physician MUST STAMP where appropriate on the form. Without the physician’s office stamp, the form WILL NOT be accepted.
Please DO NOT drop off completed physicals to front office. They will only be accepted by Athletic Director, Carlos Couzo or Athletic Trainer, Laura Cubillos.
