Please note that at the start of school all teachers are provided with the 504/IEP plans for any students in their classes.  All accommodations will be provided.  I have a copy of all plans if your child has been a student in MDCPS.  If you are transferring from out of state, I will need you to email me and/or provide a hard copy of the plan please.

Parents who are inquiring about new 504 Plans must use the below forms - Physician's Statement (1920) and Mutual Exchange to commence the process.  Provide them to me when complete.  Two meetings must occur in order for the Team to determine the student's need for a plan; an SST (Student Study Team) meeting and a 504 meeting to determine accommodations, etc.  When school starts it takes until about October/September for meetings to commence as the dates are provided by the District not MAST and District emergencies always take priority at the beginning of the year.
